Physical Examination!
Dr. May Antone is a designated Civil Surgeon by the USCIS and is highly
skilled in completing all the requirements that are needed for the I-693
Immigration Form. This includes: History, thorough physical exam, blood and
urine test and review/update full immunizations. In addition, the clinic offers:

CHEST X-RAY
- I-693 form will be completed within 7-8 business days
- Immediate appointments
Starting October 01, 2021: All applicants five years and older must provide a completed COVID-19 vaccination card per USCIS
Price for Adults
For the age of 15 and older = $300, includes:
- Immigration physical examination
- QuantiFERON TB blood test
- RPR VDRL blood test for syphilis
- Gonorrhea urine test
- A copy of the I-693 form will be given to the patient along with the sealed envelope
Price for children
For the age of 14 and under = $200, includes:
- Immigration physical examination
- QuantiFERON TB blood test
- A copy of the I-693 form will be given to the patient along with the sealed envelope
